About the Role
We have a fantastic opportunity for an Advanced Practitioner to join our team at Heath Farm. The ideal candidate will share our ongoing commitment in providing excellence in the quality placements we provide for the children and young people in our care. This is a permanent position.
Advanced Practitioners are responsible for supporting the Team Managers in the overall management of the region in order to develop practice and growth of the fostering service, ensuring a ready supply of safe, caring foster placements that offer protection to a wide range of children and young people.

Heath Farm is based in Kent and covers Kent, London and Essex.
Who we are looking for
This really is an exciting opportunity for career development as we seek a highly energetic and enthusiastic member of the team to assist with the overall management and development of service.
Heath Farm is an independent fostering agency with a holistic, wraparound care approach for children who have complex needs. We are based in Whitstable with the expectation of travel which may include Essex and London to support our carers.
Specialising in therapeutic foster care, we have a model of multi-disciplinary working and a wraparound service for looked after children.
This wraparound model of care supports children who have complex needs, while working closely with local authorities and the child’s family.
We have a highly qualified therapy team on hand to support foster carers and thestaff team. The successful candidate will need a good understanding of trauma in children and how to work with our professional foster carers and associated network to help young people settle and develop. With an outstanding team of carers and staff, who specialise in providing therapeutic care for children with complex needs, we provide four key services. These are:
- Foster care
We work alongside six independent therapists who support children affected by trauma and difficult past events. You will be working alongside our clinical lead to help children and carers get the very best out of our services.
A high level of support is crucial for foster carers. The successful candidate must be creative in thinking how support can be used and make individual to the child and carer.
Heath Farm runs the Highly specialised ‘Mockingbird’ model and as a senior member of the team, you will work alongside our Mockingbird lead to ensure all carers receive exceptional support.
- Car driver and access to a vehicle
- Current registration with the relevant Social Work regulatory council (e.g Social Work England/Wales, NISCC, SSSC)
